The challenge
What we walked into.
Pacific Health Group delivers Enhanced Care Management, Community Supports, Behavioral Health, Community Health Workers, Street Medicine, Medical Transportation and a Clubhouse program across dozens of California counties — but nothing behind it scaled. Members, caseworkers and health plan partners all landed on the same undifferentiated page. Referrals arrived by phone, fax and word of mouth with no way to track them. Outreach teams worked off spreadsheets, county launches were manual, and there was no paid, earned or organic engine bringing the model to the plans and community partners who fund it.

Whole-person brand system
Repositioned the organization around empathy, integrity and collaboration — a warm, human visual language and messaging that speaks to members, caregivers and health plan partners in their own words, extended across web, print, field collateral and recruitment.

Service architecture
Gave each of the seven programs its own page, eligibility language and calls to action, with Enhanced Care Management set as the flagship entry point that routes members into the rest of the network.

Internal systems development
Built the operating layer behind the brand: a custom referral and enrollment platform with role-based access for care teams, county-level routing, eligibility screening, document capture, status tracking and reporting the plans can audit — replacing shared inboxes, fax logs and spreadsheets.

Outreach campaigns & workflows
Designed the community and provider outreach motion end to end — territory-based target lists, field rep playbooks, co-branded partner collateral, event kits, and automated follow-up workflows so every clinic visit, county fair and plan meeting lands back in the CRM with next steps attached.

Paid media & demand generation
Launched compliance-reviewed ad campaigns across search, Meta and programmatic — geo-fenced by county and language, split between member acquisition and provider/partner recruitment, with call tracking and cost-per-enrolled-member reporting instead of vanity clicks.

Public relations & authority
Built a PR program around the model's outcomes: local and trade placements, executive positioning, county and health plan announcements, award submissions and a press-ready newsroom that gives partners something credible to reference.

Built a by-the-numbers layer sourced from DHCS and internal data, plus program education, eligibility and FAQ content optimized for how members and caseworkers actually search — so the organization ranks in every county it serves.
